How to Apply

Basic Requirements

All passport applicants must submit the following:

  • Previous US passport, if passport is not available, a certified copy of a US Birth Certificate or original US Naturalization Certificate.
  • Two (2) passport photographs. 5cm x 5cm.
  • Completed passport application. (Form DS-82 or DS-11)
  • Current government issued photo I.D.
  • Social Security number (if issued)
  • Passport fee in US dollars or Peso equivalent
Apply in Person

You must apply in person if you can answer YES to ANY of these requirements:

  • you have never had a US passport,
  • your passport has been lost or stolen
  • you were under age 16 when your most recent passport was issued
  • your most recent passport was issued more than 15 years ago.

Except for emergency of lost or stolen passports, all regular passport applications are by appointment only.  To schedule an appointment, please click HERE.

Apply by Mail

To apply by mail you MUST answer YES to ALL three requirements.

  • Can present your most recent passport,
  • Were at least 16 years old when that passport was issued.
  • That passport was issued within the last 15 years

IMPORTANT NOTE FOR CHILDREN

All children must appear in person. Both parents must execute the passport application for a child under age 16, and bring the child's birth certificate, the parent or guardian's own passport and a copy of the court-ordered guardianship. Applicants age 16 and over sign and execute their own passport applications.

Passport Photographs

Photos should be taken in everyday attire showing applicant without a hat or other head covering. Photographs may be either in color or black and white.

They must show a front view of the face on a plain light white/off-white background. For more details about the photograph Click here.

Delivery of Passports

With the exception of emergency passports, United States passport applicants will use a courier service for delivery of US passports to their home or office, saving them an additional trip back to the Embassy.

After the application is approved, the applicants will have to pay a fee in the DHL office located in the Embassy and provide the address to which they want the passport delivered.

This service will be used by all applicants who must apply for a passport in person.

Lost or Stolen Passports

If your passport has been lost or stolen present yourself at the Embassy, Colombia Ave, 4300 between Monday thru Friday 8:30-12:00. You will need to bring the following

  • Photo identification (driver's license, etc.).
  • Proof of citizenship (copy of your passport, US birth certificate or Naturalization Certificate).
  • Police report of the loss or theft.
